The "Access Improvement Grant for Maryland Children and Youth with Epilepsy” (2013-2016) funded by Health Resources and Services Administration (HRSA)was administered by the Parents' Place of Maryland in partnership with the Office for Genetics and People with Special Health Care Needs at the Maryland State Department of Health and Mental Hygiene.
The overarching purpose of the project was to improve access to comprehensive, coordinated health care and related services for Maryland Children and Youth with Special Health Care Needs, particularly with Epilepsy and other Seizure Disorders (CYSHCN-E/SD) and to increase the number of CYSHCN-E/SD who have: a medical home; an individual care plan accessible to all providers; a plan to begin taking responsibility for their own health care; and a plan for transition to adult health care. The American Academy of Pediatrics - Maryland Chapter was a partner on this project.
